Healthy Communities Foundation awards grant to D4D

D4D gratefully acknowledges a generous grant from the Healthy Communities Foundation to help us with our mission to assist those who cannot afford their medical devices. The Healthy Communities Foundation is dedicated to higher quality healthcare, greater access to healthcare, and more equitable healthcare outcomes in 27 zip codes of the Chicago area which show a 20-year disparity in life expectancy.
